Mold, floors and walls splattered with food debris and a series of violations by employees earned a Thai restaurant in Little Five points a failing health score.

The restaurant was in such bad condition, the inspector wrote “the entire facility (is) in need of a thorough deep cleaning.”

Thai 5 and Sushi Bar, on Euclid Avenue, failed its health inspection last week with a 34, according to a Fulton County Board of Health inspection report. Anything below a 70 is considered failing. Efforts to reach a manager to discuss the failing score were unsuccessful Wednesday afternoon.

Among the more egregious violations: black and pink mold-like substance was also found on the inside of the ice machine and “old encrusted food debris” was also found on the blade of a can opener and the restaurant floors, walls and shelves, according to the report.

“Unidentifiable debris” was also found in the of the vegetable prep sink.

Many employees were also unaware of when to wash their hands, according to the report. The inspector noted an employee touched their nose and neck before touching food; another employee used their cellphone then began preparing food.

Employees must wash their hands and exposed portions of their arms before any food prep, according to the Georgia Department of Public Health. They must also do so after touching bare body parts and before putting on gloves.

The restaurant was in such bad condition, the inspector recommended the entire facility be cleaned.

The inspector also noted the restaurant improperly stored food. In one case a container of raw diced chicken was stored next to “ready-to-eat” foods, including noodles, pineapples and celery.

Perhaps the least of the eatery’s worries was a broken light in the main kitchen area.

In November, received a failing score of 58; however, they passed reinspection with an 89.
