It had three new violations, including failure to properly label food containers, failure to properly cool beef and having a light in a refrigeration unit in disrepair. The restaurant is also required to designate a vegetable hand sink within 14 days.
ORIGINAL STORY (published May 15): A Vietnamese restaurant in downtown Atlanta failed a recent health inspection, with officials noting unsatisfactory "active managerial control measures."
Dua Vietnamese Noodle Soup scored a 62 following the Friday afternoon evaluation; 69 and below is considered failing. Dua's prior score, from November, was a B.
According to the report, raw food items were not properly separated inside a reach-in cooler.
Officials also reported food, including noodles and raw chicken, stored at temperatures above 41 degrees Fahrenheit.

Owner Thomas Cao said the violations shouldn’t have happened, and assures they “will never happen again.”
One of the infractions — which noted that some ceiling tiles, a piece of flooring and a “wall piece” were missing — is because the restaurant is expanding, Cao said Monday.
That area is in the hallway and no one sits there, he said.
Follow-up inspections are generally completed within 10 days of the original evaluation.
