ORIGINAL STORY: Less than two months into its existence, a Gwinnett Asian restaurant has failed its first health inspection.

Super Pho, located at 3330 Satellite Blvd. in Duluth, held grand opening festivities on May 20, according to its Facebook page. It received a 65/U after its Tuesday evaluation.

In the inspection report, officials noted a "mold/mildew like substance" in an ice machine and a lack of paper towels "at multiple sinks throughout the kitchen." One employee reportedly washed their hands in the "mop sink" and others were spotted wearing jewelry.

Several foods were also stored improperly, the report said: "Observed raw beef over ready-to-eat vegetable, observed raw bacon over cooked meat, observed raw shrimp over cooked sauce, observed raw chicken over ready-to-eat sauce in walk-in cooler."

Clean pots, cutting boards and cups were also stored in a screened-in area that was "exposed to contamination," according to the report.

Owner Mai Phan said Wednesday her restaurant is "very very clean" but acknowledged the issues noted by officials.

"We're trying to rectify all the problems and train the employees how to put the food correctly, organize it correctly," she said.

A follow-up inspection will be completed on or before July 13, the report said.
