ORIGINAL STORY (published Nov. 28): An Indian restaurant failed a recent health inspection, with officials noting that the person in charge lacked "awareness in key areas of risk factor [control] or good retail practices" for the establishment.

Amma Kitchen, located at 390 Cumming Street in Alpharetta, scored a 62/U following the Tuesday evaluation.

According to the report, there was "prepared food not marked with date of preparation or date for disposal."

Officials also reported food stores on the floor and an employee with no hair restraint.

A call with a manager was disconnected after a reporter identified herself. A call placed immediately afterward was not answered.

Follow-up inspections are generally completed within 10 days of the original evaluation.
