Five Points Healthcare, an Atlanta-based owner and operator of home health and hospice facilities, announced it has acquired Willowbrook Health Systems of Nashville. Terms of the acquisition were not released.

The deal gives the company entry into the Tennessee market where Willowbrook has operated for more than 30 years.

Five Points said it wants to become the premier provider of home health and hospice services in the Southeast. It now has facilities in Louisiana, Virginia and Tennessee.

Earlier this year, Five Points announced it had acquired BestCare Home Care, a home health provider in Virginia.
