A Wednesday afternoon inspection found "ants, roaches and flies present throughout the kitchen" of a Gwinnett County Indian restaurant, according to a report from the Georgia Department of Public Health.

Tandoor Indian Cuisine, located at 1365 Grayson Highway in Lawrenceville, received a 43/U -- its second consecutive failing grade on a routine inspection.

According to the report, employees were also observed not washing their hands after going outside and returning to the kitchen, and several food items were not stored at the proper temperature. Debris was reported on shelving in the walk-in cooler, storage containers and "shelving throughout the kitchen." In addition, "food-contact surfaces" were not sufficiently cleaned and sanitized, the report claimed.

The restaurant's permit was suspended pending a follow-up inspection within 10 days.
