LYSZKOWICZ, Jacek
With heavy hearts the Family would like to announce the passing of Jacek Lyszkowicz on October 1, 2021 at age of 74.
Known as DR Jack was born in Poland and worked in the Family Medicine in Conyers, GA. Will always be remembered and sadly missed by Family and by all who knew him.
A memorial service will be held in the All-Saints Catholic Church in 2443 Mt Vernon, Dunwoody, GA, on October 15 at 5:00 PM.
