A woman who pulled in front of another driver, causing him to swerve and fall two stories in a fatal crash, has been charged, police said.

Investigators said Tessa Mullen, 26, pulled in front of Grant Stokka Jan. 27, KCCI reported. He maneuvered to avoid a collision and flew over a fence, landing in a parking lot more than 20 feet below, KCCI reported.

"Any of us who drive know at one point or another that we have pulled out in front of someone," Des Moines police Sgt. Paul Parizek told KCCI. "It happens, it's avoidable and in this case, unfortunately, it caused the death of someone, and that is what takes it up a notch."

Stokka died. His 8-year-old son survived.

"He was hanging upside down and said he couldn't get to his seat belt, and he said, 'Mom, I just snuck out,'" Rachel Stokka told KCCI. "So, he slid out and he tried waking Grant up by kicking him, and he couldn't, so he just decided to crawl out the window."

Mullen was charged with two misdemeanors, failure to yield and reckless driving resulting in death, KCCI reported.
