A popular Dunwoody Indian restaurant failed its recent health inspection with a variety of issues.

Bay Leaf, located at 4639 North Shallowford Road, received a 39 out of a 100 on Tuesday, according to the inspector's report. Among the problems were "numerous flies"; food residue on supposedly clean cooking equipment; spices stored uncovered; and food temperature problems.

Prakash Nagarajan, an employee, said Thursday the problems had already been fixed.

Restaurants that fail are typically rechecked within 10 days.
