An online survey sponsored by the Fayette County Health Department is seeking input regarding quality of life and health concerns. The 27-question Community Health Assessment, accessible at www.surveymonkey.com/s/Fayette_CHA, asks about the current status of personal and community health, including risk factors, support services, specific needs and regional regional resources, and asks for recommendations for better care. The results will be used to develop a new community health improvement plan.
