A Caribbean restaurant in Gwinnett County failed its fourth health inspection in just over a year Wednesday, with officials reporting flies in the kitchen and improperly stored food.

Mamajuana Restaurant and Lounge, located at 950 Indian Trail Road in Lilburn, received a 61/U on the inspection. The restaurant also failed its previous routine inspection on July 25, as well as inspections on Oct. 15 and and July 27, 2015.

The restaurant's permit was suspended.

In the report from Wednesday's inspection, officials noted "multiple flies in the kitchen area," "water leaking from the fans in the walk-in cooler" and chicken and rice that were being kept at improper temperatures. Pork and ham were also being kept past their "preparation date," officials said.

Other violations included an employee drink "sitting above the salad prep cooler" and an inaccurate food theremometer.

The temporary suspension that followed Wednesday's inspection was the restaurant's third since Oct. 15, 2015. Four suspensions in a 12-month period "will result in food service permit revocation," the inspection report said.

A woman who answered the phone at Mamajuana on Friday morning said a manager was not available for comment.

A follow-up inspection will be completed within 10 days of Wednesday's evaluation.
