More than six weeks ago, a pair of dangerous Georgia inmates escaped from a prison transport bus after allegedly gunning down two prison guards.

The breakout prompted a massive manhunt before the pair were apprehended three days later in Tennessee.

On Friday, state prison officials detailed what went wrong on that rural stretch of Putnam County highway. The state Department of Corrections found that Donnie Rowe and Ricky Dubose were helped along by numerous security lapses that morning.
