The Waterford at Oakwood

This Oakwood assisted living community is operated by Capital Senior Living, which is reportedly working on a turnaround after losses earlier this year and a plunge in stock prices.

The marketing: Its website says the home’s goal is “to enrich the lives of our Assisted Living and Memory Care residents by providing an environment that is physically, mentally, spiritually and emotionally stimulating.” It goes on to talk about chef-inspired meals, a full calendar of activities and social programs, and regular off-site outings.

The reality: The facility at this address has received some of this investigation’s worst ratings. In November 2017, it was cited for abuse after a witness reported that a staff member threw water in the face of a resident with cerebral palsy and other medical issues and taunted the resident. The staff member also gave the resident an antipsychotic medication that was supposed to be discontinued. In March 2018, the home was cited for failing to ensure that 18 residents received adequate, appropriate services. These were memory care residents who are dependent on staff to administer medications. They didn’t get their medications one night when the staff member responsible for assisting with medications in memory care didn’t show up for work and a backup worker elsewhere in the facility left for the night. However, the home’s June 2018 inspection, which is the most recent one posted online, cited no violations.

Response from The Waterford at Oakwood: “As required, we reported the alleged issue to the state once it was brought to our attention. We take all matters seriously and investigated and resolved this incident accordingly. As a result, we took appropriate actions to separate our employment between this individual and the community…We are proud of the high quality care we provide at The Waterford at Oakwood.”
